Including 2-day program, catering and a ticket to the Networking Night.
Single day passes will not be offered.
Credit card payment has to be processed as part of the registration.
Payment
All amounts are in Australian dollars and are inclusive for Goods and Services Tax (GST) of 10%. Payment must be made by online credit card payment (Visa or Mastercard). All registrations must be paid in full prior to the start of the Forum, or your name badge will not be issued onsite.
Shared registrations
Please note that if you are choosing to share your registration you will only be issued with ONE name badge. We can issue the badge in your organisation’s name (e.g. DoHAC staff), you will need to notify us of this during the registration process that this registration will be shared.
Please ensure all people attending under a shared registration should be aware of the one name badge policy and bring a printed copy of the registration confirmation. Only one person under a shared registration may attend at any one time. Badges should be passed onto the next registrant or returned to the registration desk at the end of the session. Delegates must hold a badge to attend and shared badges will not be reissued if not returned.
Cancellation
Delegates who find themselves unable to attend the Forum after submitting their online registration are most welcome to nominate a substitute. All substitutions must be made by email to events@health.gov.au
Please note any accommodation or travel booking cancellations must be arranged from your point of purchase. The Department holds no liability for any losses incurred from this cancellation for travel, accommodation or any other expenses.
No Shows
If you are unable to attend, please advise us via emailing events@health.gov.au to cancel your registration. We regard your registration as 'no show' and cancellation fee of 100% will apply.
Disclaimer of liability
The Department will not accept any liability for damages of any nature sustained by participants of their accompanying persons or loss of or damage to their personal property as a result of the Forum or related events.
Insurance
The Department confirms that the delegate registration fees do not in any way include insurance cover. It is recommended that delegates consider their individual travel and health insurance requirements prior to making travel arrangements.
Privacy
As part of the registration, your personal information is being collected by the Department of Health and Aged Care (the Department). We collect personal information for the primary purpose of contacting you about, and evaluating the GMP Forum. Your personal information will only be used by relevant departmental staff for the purposes of organising and communicating with you about the Forum. This information will not be disclosed to third parties. Any information used for evaluation purposes, such as your profession or your state of residence, will be de-identified and will only be disclosed in aggregate. We will not disclose your personal information to any overseas recipients. You can read the department’s privacy policy here. Media and photography
In registering for the Forum, you accept that various events may be photographed, video or audio recorded and that this content may be used by the Department at its discretion on website, social media, newsletters or other promotional material.
